After Ayushman Bharat Yojana (AB-PMJAY) was launched in West Bengal on 16 August 2026, about 1.5 crore Swasthya Sathi families of the state are gradually migrating to the central scheme. This article shows — how to do the Ayushman card verification, e-KYC and download online from home, and which rules now apply at hospitals.

First, one thing: although this is called a “port,” no separate form actually has to be submitted — if your name is on the NFSA/Swasthya Sathi list, that data itself comes over to PM-JAY; you only have to complete e-KYC and generate the card.

The Core Conditions of Migration

  • The family must be on the NFSA (Khadya Sathi) list.
  • Each member’s Aadhaar card must be linked with the mobile number.
  • If e-KYC is not completed, the card will not be printed.
  • A separate Ayushman card is created for each member (unlike Swasthya Sathi’s single family card).
  • Coverage is the same: ₹5,00,000 cashless per family per year.

Downloading the Ayushman Card Online: Step by Step

Method 1: Web portal

  1. Open beneficiary.nha.gov.in in your browser.
  2. Log in with OTP using the mobile number linked to your Aadhaar.
  3. Choose ‘Search Beneficiary’ and select the state (West Bengal), district and scheme.
  4. Search by Aadhaar number / ration card / family ID — if your name is on the list, the family will appear.
  5. Complete e-KYC for each member (with Aadhaar OTP).
  6. After approval, click the Download Card button to download and print each member’s Ayushman card PDF.

Method 2: Mobile app

Download the Ayushman App (official PM-JAY) from the Google Play Store and complete the same steps — OTP login → search → e-KYC → download — on the mobile itself.

Method 3: Hospital help desk

If you don’t have a card, submit your Aadhaar at the Ayushman–Swasthya Sathi desk of any of the state’s more than 1,300 empanelled private hospitals or a government hospital and fill out the enrolment form — after verification, the card will be created (under Ayushman or MMSBY).

The New Hospital Treatment Rules (SOP 2026)

Under the health department’s new SOP, at the time of hospital admission:

  • Aadhaar will be used to first check whether the Ayushman card has been created — if so, treatment will take place under PM-JAY.
  • If e-KYC is pending, the hospital will attempt e-KYC on the portal; if there is a problem, treatment will proceed on the Swasthya Sathi card.
  • If there is a technical glitch on the PM-JAY portal, treatment can continue on Swasthya Sathi after submitting evidence with screenshots.
  • Patients over 70 must first be enrolled in Ayushman Vaya Vandana (VVS).

In other words — once you have the Ayushman card, that becomes the primary card; Swasthya Sathi remains as a backup.

What If You Are Not Eligible for Ayushman?

If your name didn’t come onto the PM-JAY list even though you were on Swasthya Sathi, the state government’s MMSBY scheme is for you. It too offers Rs 5 lakh of cashless benefit a year, with portability across India. Details: Apply Online for the MMSBY Scheme and MMSBY eligibility rules.

Questions People Ask About This Scheme

Is there a separate fee to 'port' from Swasthya Sathi to Ayushman?

No. The migration and Ayushman card creation are completely free. There is no need to pay anywhere other than the beneficiary.nha.gov.in portal, the official Ayushman App, government camps or a hospital's help desk.

My name is not on the NFSA/Swasthya Sathi list — will I get an Ayushman card?

Not directly on the portal if your name isn't listed, but you can submit your Aadhaar at any empanelled hospital and fill out the enrolment form — after government verification, a card is created under Ayushman Bharat or MMSBY.

After getting the Ayushman card, do I have to surrender the Swasthya Sathi card?

No, there is no rule to surrender it. However, under the new SOP, once the Ayushman card is created, treatment at hospitals will generally take place under PM-JAY. Keep the Swasthya Sathi card with you as a backup.

My mobile number is not linked to Aadhaar — what do I do?

First get the mobile number updated at the nearest Aadhaar Seva Kendra. Without OTP verification, e-KYC and card download are not possible.
